It is important to report your lost passport as quickly as possible to protect your identity.

You can report your lost or stolen passport online, by phone or by mail.

Call the U.S. Department of State via its toll-free line at (877) 487-2778 or TTY (888) 874-7793.

If you need to apply for a new U.S. passport, you must complete Form DS-64 with your application.

How to Replace a Lost or Stolen Passport

ATTENTION:

  • If you are in the United States and traveling in 2 weeks or less, make an appointment at a regional passport agency or center to replace your passport in person.
  • If you are in the United States but not traveling within 2 weeks, you can make an appointment at an authorized passport acceptance facility.
  • When you are outside of the US, contact your nearest U.S. embassy or consulate.

The fees to replace a lost or stolen passport are as follows:

  • U.S. Department of State Fee: $110.00 (approx.), depending on your age.
  • Acceptance Agency Fee: $25.00
  • Expedited Fee: $60.00
