How to Complete the DS-5504 Passport Form

After changing your name through marriage, divorce, or a court order, you’ll need to apply for a new passport.

Printing Errors and Spelling Errors

Correcting these types of errors is free while your passport is still valid.

  • If reported within 1 year of issuance (no fees): Your corrected passport will be valid for 10 years from the new issue date.
  • If reported after 1 year of issuance (no fees): Your corrected passport will be valid until the expiration date of the original, incorrect passport.

Passport Issued Less Than 1 Year Ago

You may use DS-5504 Passport Form to submit a name change, data correction, or replace your limited passport book. You must submit Form DS-5504 by mail with the following:

  • Your current passport.
  • Your name change document.
  • A color passport photo.

Passport Issued More Than 1 Year Ago

If your passport was issued more than 1 year ago and you meet the following requirements, you may simply renew your passport:

  • Your current passport must have been issued less than 15 years ago.
  • You were older than 16 years old when you obtained it
  • You have your passport in hand (it is not lost, stolen, or damaged).

You’ll need to complete and submit Form DS-82 by mail with:

  • Your current U.S. passport.
  • Your name change document.
  • A color passport photo.
  • All applicable passport fees.
