Passport applicants with a gender reassignment in progress (or completed) must present a series of documents with the passport application.

Gender Reassignment Passport Applicants

Importantly, the documents and passport photos you present with your U.S. passport application must reflect your current appearance. Otherwise, the U.S. Department of State may not accept it.

Also, you must submit a physician’s certificate with your passport application, which allows the passport acceptance facility to issue a new U.S. passport in your new gender. Furthermore, your physician’s certificate should also indicate whether your gender transition is in progress OR complete.

NOTE: Moreover, if you changed your name, you must also submit evidence of the legal name change.

Completed Gender Reassignment

Important to realize, if the physician’s certificate indicates that your gender reassignment is complete, you are eligible for a U.S. passport with 10 years of validity. Also, it must be original, signed, and on an official letterhead.

Letter Requirements for the Physician’s Certificate:

  • The letter must include all of the following:
    • Physician’s full name, medical license or certificate number.
    • The issuing state or jurisdiction of medical license or certificate.
    • The Drug Enforcement Administration (DEA) registration number assigned to the physician.
    • The address and telephone number of the physician.
  • Next, it must show there is a doctor/patient relationship.
  • Then, it must also indicate you had the appropriate clinical treatment for the gender reassignment.
  • Finally, it must conclude with, “I declare under penalty of perjury under the laws of the United States that the forgoing is true and correct.”

In-Progress Gender Reassignment

Moreover, if the physician certificate states the gender reassignment is in progress, you are eligible for a passport with limited validity of 2 years.

Letter Requirements for the Physician’s Certificate:

  • It must be original, signed, and on an official letterhead.
  • Also, includes your physician’s full name, medical license or certificate number (and issuing state).

Of course, you may extend a limited passport book to the full 10 years at no fee by submitting Passport Form DS-5504 within 2 years of its issue date.

I, (Physician’s Full Name), (Physician’s Medical License or Certificate Number), (Issuing State of Medical License/Certificate), (DEA Registration Number), am the attending physician of (name of patient), with whom I have a doctor/patient relationship.

(Patient’s Name) has had the appropriate clinical treatment for a gender reassignment (specify new gender).

OR

(Patient’s Name) is in the process of a reassignment to a new gender (specify new gender).

I declare under penalty of perjury under the laws of the United States that the forgoing is true and correct.
