Use the DS-5504 Passport Name Change Form to submit a name change, data correction, or replace a limited passport book.

Change or Correct a U.S. Passport

Printing Errors and Spelling Errors

NOTE: Presently, the requirements for corrected passports are the same for adults and minors.

You may correct printing errors and spelling errors at no charge while your passport is still valid, which includes an incorrect name, gender, or birthplace.

  • If reported within 1 year of issuance, you will get a new corrected passport. It is valid for 10 years from the issue date of the corrected book.
  • If reported after 1 year of issuance, your new, corrected passport will be valid until the expiration date of the original, incorrect passport.

To correct a printing or spelling error, you can simply mail in the U.S. Passport Name Change, Data Correction, and Limited Passport Book Replacement (Form DS-5504) along with 1 acceptable passport photo and evidence of the error. Also, see Passport Name Change for more details.

DS-5504 Passport Name Change Form Instructions

Subsequently, you must apply for a new US passport once you change your name through marriage, divorce, or a court order.

  • If you received your passport less than 1 year ago, you will not have to pay additional fees for the name change unless you want it expedited.
  • If your passport was issued more than 1 year ago, you may submit a passport renewal application. This is only available if:
    1. The U.S. State Department issued the passport less than 15 years ago.
    2. You were over 16 years old when you received it.
    3. Your passport is in your possession (it is not lost or stolen, or damaged).
