Use the DS-5504 Passport Name Change Form to submit a name change, data correction, or replace a limited passport book.

Change or Correct a U.S. Passport
Printing Errors and Spelling Errors
NOTE: Presently, the requirements for corrected passports are the same for adults and minors.
You may correct printing errors and spelling errors at no charge while your passport is still valid, which includes an incorrect name, gender, or birthplace.
- If reported within 1 year of issuance, you will get a new corrected passport. It is valid for 10 years from the issue date of the corrected book.
- If reported after 1 year of issuance, your new, corrected passport will be valid until the expiration date of the original, incorrect passport.
To correct a printing or spelling error, you can simply mail in the U.S. Passport Name Change, Data Correction, and Limited Passport Book Replacement (Form DS-5504) along with 1 acceptable passport photo and evidence of the error. Also, see Passport Name Change for more details.
