Passport Name Change Checklist

In order to complete a name change on a U.S. Passport, you must complete the following checklist. Any errors on your application will delay the processing of your new passport. If your passport was issued less than one year ago, please submit a passport renewal application. If you do not need a name change passport in two weeks or less, click here for information on routine passport processing. Step 1: Gather Your Documents

 DOWNLOAD PASSPORT LETTER OF AUTHORIZATION (click here)
 SUBMIT PASSPORT PHOTOGRAPHS
 SUBMIT VALID PASSPORT
 PROOF OF NAME CHANGE
  • To prove your name change, you are required to provide official documentation certifying the change. The two official documents accepted are a Marriage Certificate or a Court Order.
  • The name change document MUST bear the official seal of the issuing authority.
  • Uncertified copies or notarized documents are UNACCEPTABLE.
 SUBMIT PROOF OF INTERNATIONAL TRAVEL
 GOVERNMENT FEES:
  • U.S. Department of State Fee: $60.00 Make check or money order payable to U.S. Department of State. Write your date of birth on the check or money order
  • IMPORTANT: If payment is made incorrectly, the passport agency will deny your passport application. Click here for Passport Fees Explained.
  • STARTER CHECKS AND 3RD PARTY CHECKS WILL NOT BE ACCEPTED
  COMPLETE THE APPLICATION FORM (DS-5504) (click here)
  • All passport agencies now require the use of this online form. Complete this form online. The last page will ask for payment type. Since you are using an expedited service, you do not need to worry about this payment. Simply select any option and continue. Scroll to the bottom of the last page and click “Create Form” so a new .pdf file will open in a new window. This is the barcode application required by the U.S. State Dept. You do not need to make any payments online for this form. Just create the form so you can print it out.
  • Fill out the DS-5504 (Name Change Application form) online.
  • Print out completed application form.
  • Sign and date application form.
